Renewal of the accreditation for family planning centre
A renewal questionnaire will be sent six months before accreditation expires. It must be returned within 30 days with specific documents, including those from Article 3 and a new fire brigade report if needed. Upon meeting these conditions, provisional renewal is granted pending Ministers' decision.

No later than six months before the expiry of the period of validity of the accreditation, a questionnaire will be sent by the administration to the centre's organising authority with a view to renewing the accreditation.
Required documents:
This questionnaire must be returned, duly completed and signed, within 30 days of its receipt, accompanied by the following documents
1. those referred to in Article 3, paragraphs 1 and 7;
2. those referred to in Article 3, paragraphs 2, 3, 4 and 8, if any changes have been made;
3. a new report from the fire brigade:
(a) where the previous report was prepared more than five years ago;
(b) when the buildings or equipment have been modified in a way which could affect the safety of the centre.
§ 2. When these conditions are met, the accreditation will be provisionally renewed until the decision of the Ministers.
